At The Restore Foundation, we believe that financial barriers shouldn’t keep people from receiving the care they need.

We’re currently building a process to help connect individuals with financial assistance for care. While that process is still in development, we invite you to complete the form below to let us know you’re seeking support.

Once the process is available, requests will be considered in the order they were received. We don’t have a confirmed timeline for when the process will be ready, but submitting your information now helps us prepare to respond to your request when it is.
